Around 40 types infect the anogenital tract. About a dozen are classified as high-risk (oncogenic). The rest are low-risk and cause warts or nothing at all. Persistent infection with a high-risk type is what causes cancer, and persistence is more common in smokers, men with HIV or other immunosuppression, and men with multiple concurrent HPV strains.<\/p>\n<h2 id=\"transmission\">How men catch HPV<\/h2>\n<p>HPV spreads by direct skin-to-skin or mucosa-to-mucosa contact, primarily during vaginal, anal, and oral sex. Condoms reduce but do not eliminate transmission because the virus lives on skin outside the area a condom covers. When symptoms do appear, the most common presentations are:<\/p>\n<p>An office-based option worth mentioning is <a href=\"\/tr\/glans-penis-head-filler-in-the-treatment-of-premature-ejaculation-op-dr-cem-ipek\/\">glans (penis head) filler for premature ejaculation<\/a>.<\/p>\n<ul>\n<li><strong>Genital warts<\/strong> \u2014 soft, flesh-coloured or slightly darker growths, sometimes cauliflower-shaped, sometimes flat and papular<\/li>\n<li><strong>Itching or mild irritation<\/strong> of affected skin<\/li>\n<li><strong>Bleeding<\/strong> from a wart that gets caught during sex, shaving, or wiping<\/li>\n<li><strong>Anal warts<\/strong> that feel like small lumps at the anal margin, sometimes with itching or spotting<\/li>\n<li><strong>Oral warts<\/strong> \u2014 small painless bumps on lips, gums, tongue, or throat<\/li>\n<li><strong>Persistent sore throat or hoarseness<\/strong> lasting more than 2\u20133 weeks (rare but a red flag for oropharyngeal cancer)<\/li>\n<li><strong>A firm lump or a non-healing ulcer<\/strong> on the penis (rare, warrants urgent evaluation)<\/li>\n<\/ul>\n<p>HPV itself does not cause pain, discharge, burning during urination, or fever. Type 18 causes most of the remainder. Genital warts almost never carry oncogenic types; a patient with warts is not at elevated cancer risk from those particular warts, but concurrent high-risk infection is common and cannot be detected visually.<\/p>\n<p>A nerve-targeted alternative when medication fails is <a href=\"\/tr\/selective-dorsal-cryoablation-for-premature-ejaculation\/\">selective dorsal cryoablation for premature ejaculation<\/a>.<\/p>\n<h2 id=\"penile-cancer\">HPV and penile cancer<\/h2>\n<p>Penile cancer is rare in developed countries (roughly 1 case per 100,000 men per year), but HPV drives about 40 to 50 percent of cases and up to 80 percent of the specific basaloid and warty subtypes. Warning signs include a firm, non-healing ulcer, a red velvety patch on the glans, a persistent scaly lesion, thickened foreskin that will not retract, or unexplained bleeding. Risk is highest in men who have sex with men (MSM), especially those living with HIV, in whom rates approach 130 per 100,000 (comparable to historic cervical cancer rates before screening). High-resolution anoscopy and anal Pap smears are offered in specialist clinics for HIV-positive MSM and other high-risk groups.<\/p>\n<h2 id=\"oropharyngeal\">HPV and oropharyngeal cancer<\/h2>\n<p>This is the fastest-growing HPV-related cancer worldwide and now more common in men than cervical cancer is in women in the United States. Roughly 70 percent of oropharyngeal cancers in the US are HPV-driven, almost all by type 16. Symptoms are subtle: a persistent sore throat, a lump in the neck (metastatic node), unexplained ear pain on one side, or trouble swallowing lasting more than 3 weeks. The male genital surface is much larger, keratinised (thicker) in most areas, and yields inconsistent samples that produce many false negatives. Additionally, most male HPV infections clear on their own without ever causing cancer, so a positive result creates anxiety without changing management. For those reasons the FDA and CDC do not endorse a general male HPV test.<\/p>\n<p>Adults can still benefit \u2014 see <a href=\"\/tr\/hpv-vaccine-for-adult-men\/\">HPV vaccine for adult men<\/a>.<\/p>\n<h2 id=\"when-tested\">When testing is offered<\/h2>\n<p>Testing is used selectively in the following situations:<\/p>\n<ul>\n<li><strong>Anal Pap smear<\/strong> \u2014 HIV-positive MSM, transplant recipients, men with a history of anal warts<\/li>\n<li><strong>High-resolution anoscopy<\/strong> \u2014 abnormal anal Pap or persistent perianal disease<\/li>\n<li><strong>Biopsy<\/strong> \u2014 any wart that is atypical, pigmented, bleeding, ulcerated, or resistant to treatment<\/li>\n<li><strong>Urethral meatoscopy<\/strong> \u2014 suspected urethral warts<\/li>\n<li><strong>Oral \/ oropharyngeal exam<\/strong> \u2014 persistent sore throat, neck node, or unilateral tonsillar enlargement<\/li>\n<\/ul>\n<h2 id=\"partner-positive\">If your partner has tested positive for HPV<\/h2>\n<p>This is one of the most common reasons men come to see me. The short version: your partner testing positive does not tell you anything actionable about your own status, because you have almost certainly already been exposed to the same strain. The vaccine is licensed for men up to age 45 in the US (FDA 2018 expansion). Vaccine efficacy against persistent infection with vaccine-covered types exceeds 90 percent in previously unexposed men and remains meaningful after exposure. Dr. Cem \u0130pek<\/h2>\n<blockquote><p>Most of the men I see in Istanbul for HPV are healthy, in their thirties, and either found a wart on the shaft in the shower or received a message from a former partner. Both groups feel humiliated. My first job is to explain that HPV is not a moral failing; it is the most common STI on Earth, and eighty percent of adults meet it during their lifetime.<\/p><\/blockquote>\n<blockquote><p>The bigger clinical concern is not the wart itself but the possibility of a coexisting high-risk strain. I always examine the anal margin, palpate the neck for enlarged nodes, and ask about oral symptoms. Ninety-five percent of the time everything else is fine, but the five percent where I find an early oropharyngeal tumour justifies the two-minute check.<\/p><\/blockquote>\n<blockquote><p>The men who come back years later with recurring warts almost always share three things: they were never vaccinated, they smoke, and they only ever had freezing done in fifteen-minute clinic slots with no follow-up topical treatment.
